Sessions 1 & 2 – Navigating Your Health Center’s Scope of Project
Before you hit the trails, make sure your map is accurate! For community health centers, Forms 5A, 5B, and 5C chart the course for your approved scope of project—defining your services, sites, and other key activities. Inaccurate or outdated forms can lead to serious consequences, including grant conditions and denial of Federal Tort Claims Act (FTCA) coverage.
This two-part session will begin with a deep dive into Form 5A, including how to add or remove services and avoid common pitfalls in change-in-scope requests. We’ll also review a practical checklist of required elements for Column II contracts and Column III referral arrangements. Next, we’ll explore Form 5B, focusing on what qualifies as a “service site” and walking through best practices—and frequent missteps—when adding or removing sites, illustrated by real-world examples. Finally, we’ll unpack Form 5C, clarifying when it should (and should not) be used. Get ready to build your scope-savvy survival skills and keep your health center on the right path.
